The services described here offer educational support for Our Members who have certain chronic health care conditions or may want to stop using tobacco, lose weight or need nutritional counseling. Our team of specially trained nurses, health educators and registered dietitians are here to help. We may change them at any time as necessary to meet the needs of Our Members.

HEALTH MANAGEMENT

Molina Healthcare offers programs to help You and Your family manage a diagnosed health condition. Our programs include:

  • Molina My Health Chronic Obstructive Pulmonary Disease (COPD) management
  • Molina My Health Substance Use Disorder
  • Molina My Health Depression
  • Molina My Health Heart Failure
  • Molina My Health Hypertension
  • Molina My Health Tobacco Cessation
  • Molina My Health Nutrition Consult
  • Molina My Health Weight Management

You can also enroll in any of the programs above by calling the Molina Health Management Department at
833-269-7830 9:30 a.m. and 5:30p.m. (PST), Monday through Friday.

Smoking Cessation Program

Molina partners with the California Smoker’s Helpline to offer telephonic smoking and vaping cessation counseling. It is available to all members. Pregnant smokers will need a doctor’s approval. Counseling is made available in six languages. Nicotine Replacement Therapy is covered with a doctor’s prescription. It is at no cost to you. You can enroll by calling 1-800-NO-BUTTS or 1-844-8-NO-VAPE.

Diabetes Prevention Program

 

Molina has teamed up with Yes Health to bring you the Diabetes Prevention Program (DPP). It is at no cost to you. Getting and staying healthy is easier and more fun. Yes Health is an all-mobile program. It includes coaches who provide one-on-one nutrition, fitness, and well-being advice. Take the quiz on their website to see if you qualify. You can then enroll. Stay healthy this flu season. Everyone six months and older should get the flu shot.
